Certified Nurse Assistant Pre-Enrollment Requirements

REQUIREMENTS FOR ENROLLMENT

MEET PRE-REQUISITES

Must have computer and internet access to complete the online portion of the class.

TWO PERSONAL DOCUMENTS

Current state-issued photo ID and Social Security card

IMMUNIZATION REQUIREMENTS OR EXEMPTION REQUEST*

Tuberculosis - Report of Tuberculosis (TB) Skin Test OR TB Chest X-Ray Report).​Must be completed within the past 6 months. Information about testing can be found on 

Influenza (Flu) Shot - Required during flu season in the fall and spring (not required in summer courses). Information about the flu vaccine can be found on the .

Evidence of COVID-19 Vaccine

Refer to the CDC’s Recommended  OR for additional guidance. Additional immunizations might be necessary in order to complete the internship(s). 

READING TEST OR COMPLETION OF A COLLEGE-LEVEL COURSE

Successfully complete one of the following reading requirements and meet the applicable score:

If you have taken a college-level writing course (English Composition, Expository Writing, etc.) and passed with a grade “C” or higher, you will need to submit your transcript or grade report.

*Students may seek a medical or religious exemption from a clinical partner's vaccination requirement. Refer to the Vaccination and Exemption Webpage for more information.
